When selecting Forms from the Patient Navigation Menu, all forms previously completed will display.

Forms displayed, previously completed, can be viewed by clicking on the name of the form in blue, or by clicking the blue ‘view’ box to the right of the form title.  Once the visit is closed, certain forms are no longer “editable,” and you will no longer see the blue ‘edit’ button.

To fill out a new visit record:  Select Fill out Form in the Patient Task Bar.

All possible forms for the selected visit type will display (for example, if a routine visit type is opened, an initial nursing assessment or initial psychosocial assessment or initial spiritual assessment will not be displayed as an option to select and complete).  If a form needed is not displaying, check the selected visit type is correct.  If the visit type is correct, notify Hummingbird Support as this might indicate a form needs to be re-mapped.

NOTE: Visit types are mapped to ‘Job Description/Role;’ Forms are mapped to ‘Visit Type.’  In the example below, this person is both a nurse and SW; therefore, she will access both nursing and SW visit types and nursing and SW forms.

It will display: Edit – (and the form name).

Once the form is complete, the user should preview the form (there is NO SAVE button in this program).

Functions and Features of Forms

Required Questions:

If these questions are not all answered before ‘previewing’ or ‘filling out an additional form’, an alert will notify the user that ‘All Required Questions have not been Answered.’

Select Finish Now, and the program will take user directly to the first unanswered Required Question.  For multiple unanswered required questions, the program will continue to search and find all unanswered required questions, OR you can select NEXT UNANSWERED REQUIRED from the Patient Task Bar.

Select Finish Later, and the program will display the Forms page (all forms previously completed).  The incomplete form will display as follows:

NOTE: It will not be possible to close this visit until all required questions are answered.

When filling out a form, it is NOT necessary to answer every question.  Answer only what is pertinent to the patient, i.e. if an assessment of the cardiac system was not done on this visit, do not answer any of the questions.  The only information that appears on the patient’s visit record is the information associated with the questions you answered.

Answering questions with circles (radio buttons ¤) indicate only one answer can be selected.  If the user makes an attempt to select multiple answers, the first selected answer will be de-selected.

Answering questions with a checkbox  indicate multiple answers can be selected.

Exception, if one of the answers is ‘normal’.  If ‘normal’ is selected as well as ‘anxiety’, the ‘normal’ will automatically be de-selected.

Some questions when answered will trigger an additional question, child question. The question triggered by the initial question may or may not be required.

NOTE:  Donation was selected, an additional question or information is asked.

Sticky Questions

Throughout the program, in many forms, care plan problems and orders, the word ‘Sticky’ will display in white & bold on questions and responses. This feature has 2 functions; Informative and Time Saving. This feature allows some responses to pre-populate the response to subsequent forms/orders/care plans containing the same question.  In other words, when a ‘Sticky’ question is answered, that response will be seen on another form, order or care plan that contains that exact same question.  When opening a form/order/care plan populated with ‘Sticky’ responses, the user is seeing the most recent response to that question according to the VISIT DATE & TIME.

When completing the Form, it’s important to pay close attention to the responses that are ‘Sticky’ and making any and all applicable changes to reflect the current assessment or evaluation. Click PREVIEW from the Patient Task Bar.

A pop-up box will display; ‘There are sticky questions that you have not changed’. Select Review Later, Accept or Review.

If ACCEPT is selected, the worker is accepting accountability for the accuracy of the documented assessment or evaluation, and the form will preview.  If Review is selected, the form will display ONLY ‘Sticky’ questions with responses that were NOT changed.  If REVIEW LATER is selected, the form will display, however, the visit cannot be closed until the ‘Sticky’ Review is completed.

Generally, questions are sticky from the Initial Assessments (Nursing, Social Worker, Spiritual) to the Visit Record for that discipline and from Visit Record to Visit Record.  As seen in the Examples 20, 20a & 20b above, some questions stick from the Initial Nursing Assessment to the POC/Comprehensive Assessment-Initial.  For a complete guide to the sticky feature, refer to the Tips Sheet, ‘Stickies’.

If the wrong form has been selected from the list of forms, when the form opens, you may select Undo from the Patient Task Bar.

A Pop-Up Box Alert will display asking for confirmation of selected Undo Task

The correct form can be accessed by selecting Fill out form from Patient Task Bar.

Amending (Correcting) a Form once the visit has been closed:

Most documents completed in the form section of the program are not editable after the visit is closed (you cannot make changes to the actual document). However, in the event an error or omission in documentation occurred, an amendment can be added by using the right pane or large box on the right side of the screen. First in the small box above the large box that says Take General Notes, click on the down arrow in this field. The drop-down list of items will include a list of all forms completed in white.  Select the form that needs to be amended.
Screen Shot 2019 01 23 at 11.44.18 AM

The small box above the large box will now show: Add addendum to: ‘the selected form’. Add the addendum in the large box and the newly added addendum will display in gold in an Addendums section at the bottom of the Form.

For example: the patient reported pain in left shoulder not right shoulder. Note that you do not have to enter name or date. The entry will be stamped with writer’s electronic signature and date.Screen Shot 2019 01 23 at 11.44.06 AM
