The first IDG/IDT while using HBC is always a bit awkward; however, preparation and organization will smooth out the process. All documentation from the team members should be complete PRIOR to the meeting.
First Electronic IDG/IDT
The “Team Meeting” process may require additional training and practice. (See H4.4 “Medical Records”)
Our recommendation is that one person be assigned to open the applicable “team meeting order type”, i.e. Plan of Care/Comprehensive Assessment Recert or Update (the Plan of Care/Comprehensive Assessment Initial will be opened by the admitting nurse), for each patient scheduled to be discussed at the first “Electronic IDT” meeting. Label with the date of the IDG/IDT Meeting on Label Line at the top of the Order.
Prior to IDG/IDT, all disciplines should document on the POC/CA Initial/Recert or Update (under Summary of Needs Section, by discipline); add any notations regarding care plan changes and summary of care since the last IDG/IDT.
By the time of the IDG/IDT meeting, POC/CA Updates, and / or Recertification Summaries should be complete.
We recommend that a multi-media projector be used during IDG/IDT so that all team members can view each patient’s plan of care. Consider having one person lead the meeting and another act as “scribe” to enter any recommendations from the Team.
After the meeting, and after all necessary comments are added, an assigned person will “Resolve” the team meeting orders. These can then be printed and distributed to the PCP and SNF, etc. if necessary.
Use of Computers at IDG/IDT
There should be only two computers being used at the IDG/IDT meeting.
Although all team members may bring their laptops to the meeting, it is strongly suggested that only 2 laptops are used to manage/moderate the meeting.
Leader of the meeting
This team member will have laptop/desktop attached to projector; will lead the discussion.
Leader of the Meeting (during the meeting):
There is no need to open visits. Connect laptop/desktop to projector.
Choose the first patient for discussion (use the Team Meeting Report from PIM). Click on “ORDERS” and choose the IDG/IDT document labeled for today’s meeting.
In right-hand pane, “Care Plan Notes”, click the down-arrow. Under “Patients”, click on current patients.
You can easily move to the next patient for discussion.
Scribe
This team member will have laptop/desktop and will open visits (see below) so that any recommendations made by the team can be entered into the program. Will also prepare for the next IDG/IDT. If a scribe is not used, then individual team members will be responsible for updating the document either during the meeting (if allowed by admin) or within 24 hours after the meeting.
Scribe (before the meeting):
Log into program
Select Open All Icon in top Tool Bar. Select the group of patients you will be discussing (don’t forget to include new admissions, deaths and recertifications).
Use the Team Meeting Report from PIM Reports to help in choosing patients for IDG/IDT.
You may also wish to have a “quick-selector” of “Team” added to the Open All page.
Do this in PIM/Admin/Persons and Places/IDG Teams.
Add your team names (such as Red, Blue, etc.). Once this is done, you will see on the Open All page (in addition to recent referrals and assigned) with quick-selectors of your team names.
Choose visit type of IDG/IDT Visit.
You will see all of your open visits listed, in yellow. Choose the first patient called by the Leader of the meeting.
Scribe (during the meeting):
In the right pane of the screen: Care Plan Notes Pane, click on the arrow down to display options. Choose “Open Visits”. You will now see all open visits listed in the right-hand pane and you will be able to choose each patient as he/she is called up for discussion.
For each patient, click on ORDERS in the bottom Navigation Menu.
Find and select the appropriate IDG/IDT document for this meeting i.e. IPOC, POC/Comprehensive Assessment Update or Recert which is labeled with the meeting date.
As recommendations are made by the Team, click EDIT, and select “N/A to stickies”, then add the recommendations, and whether or not patient is appropriate for continued services.
Print out POC/CA Recertification to send to the attending physician if necessary.
Scribe (after the meeting):
Current meeting’s IDG/IDT documents will need to be “resolved” and a new one opened for the next IDG/IDT. To do this:
- Go back into each patient’s records, click “ORDERS” and choose the IDG/IDT document that was just reviewed. IF ALL COMMENTS HAVE BEEN ADDED AND ALL DISCIPLINES HAVE ENTERED THEIR INFORMATION, “resolve” the order. This can be done during the meeting if you wish.
- Choose “Fill out order,” answer “N/A” to stickies, and choose the appropriate document for next IDG/IDT (either the Update or the Recertification).
- On the “label line”, enter the date of the next IDG/IDT in which this patient will be reviewed (usually 14 days away). Enter the appropriate certification period on the line, as well as dates of the next review.
- Close the visit or go to next patient and “Close All’ after all are complete.
- Repeat steps 1-4 for each patient reviewed.
