The HBC Default Data Set, from which your data set is built, contains a form named “Incident/Complaint Form.” mumms Software would like to stress the proper use of this form and below we offer recommendations on the use of the form.

It is the opinion of mumms Software that Incident Reports and/or Complaint Reports should not be a part of a patient’s medical record. However, in order to provide field staff with a mechanism to easily report such incidents or complaints, mumms has created a form for such use.

Option 1: (Incident/Complaint Form is Never in the EMR)

  1. Create a fake patient w/ the “name” Incident/Complaint. Staff needing to report an Incident or Complaint will navigate to that fake patient, and will complete the form, entering the real name of the patient or employee involved, along with details. The form will be emailed or printed for the Supervisor.
  2. Supervisor will investigate; enter findings into the form (the form is “editable”) and resolve when completed. Form can be printed for analysis and monthly/quarterly summation. Note: At this time, there is no report generated from the form. Note: The form is set to archive after 5 days so that it is unavailable for staff to view unless the chart is unarchived.

Option 2: (Incident/Complaint Form is Temporarily in the EMR)

  1. Staff needing to report an Incident or Complaint will, from the patient’s EMR, navigate to “Forms” and choose the Incident/Complaint Form and enter details of the incident or complaint. The form will be emailed or printed for the Supervisor.
  2. Supervisor will investigate; enter findings into the form (the form is “editable”) and resolve when completed. Form can be printed for analysis and monthly/quarterly summation. Note: At this time, there is no report generated from the form.
  3. Supervisor will then delete form via the Admin Console or can move the form to the fake patient mentioned above (named Incident/Complaint).

Visit Type – Two Options for Accessing the Incident/Complaint Form:

Option 1: Form is accessible from any visit type. Benefit is that the form is very easy to find.

Option 2: A special visit type of “Occurrence” can be set up in PIM and the Incident/Complaint Form can be mapped the new visit type only. Benefit: Supervisor is able to run a report of visit types and ensure that all incidents and complaints for a time period have been received.
