New Physician Order, Plan of Care/Comprehensive Assessment Initial Orders and Admitting Orders will be populated with all new medications, discontinued medications and modified medications added or changed during this visit.
New Physician Orders
Once a medication is added to the Medication Profile, the nurse should, within the same visit, open a New Physician Order. The New Physician Order will display the medications immediately. If there were any medications ordered that Hospice IS NOT covering, then a large box would display for the physician to add notes related to non-coverage. If there is any ‘Required’ information, complete it.

Note the signature area at the bottom of the order. When selecting Hard Copy Signature, it displays REQUIRED questions for date and signature. The ‘required signature’ function is different than the REQUIRED function described earlier.

This function allows the signature and date lines to PRINT in the PREVIEW format. (Nurse may wish to type in the physician name in the line ‘Print name signature’). The order will preview with signature line and date line for the physician to complete.

NOTE: If the nurse closes the visit without completing the New Physician Order, then she/he must reopen a visit and choose “Medication Order”. All meds will need to be manually entered into the “Medication Order.”
****TIP*****ALWAYS enter your new medication on to the Med Profile, then go to New Physician Order while in the same visit. This will save a lot of time.
Plan of Care/Comprehensive Assessment type order
(Used for IDT/IDG documentation): Enter all summary and progress notes associated with discipline type. When finished, click on Preview in the Task Bar; Plan of Care/Comprehensive Assessment Order will display with Meds and Care Plan Problems (when in Preview format) as well as any questions answered, or summary notes taken.
