Visits

The Visits section is primarily for auditing/viewing documentation. To view visits, select Visits from the Patient Navigation Menu.  All Visits (made to the HBC chart) will display in visit order.  All Forms, Care Plan Problems and Orders done during visit will display.  Any Notes taken during the visit will display here as well.

NOTE: the term ‘Visit’ is referring to a ‘visit to the chart.’  This section displays all persons who ‘opened a visit’ in this patient’s chart as well as displays the documentation, which was completed during the visit.  If a visit shows NO documentation, the chart may be ‘archived.’

To view the details of the visit, click on the Visit.  All Forms, Care Plan Problems, Medications, Orders and Notes will view in their entirety.

Notes

The Notes section is primarily for reading updates to care plan problems and addendums to visit records.  Documentation cannot be added to this section directly.  Notes that display in this section are notes that are entered in the right pane. To view Notes, select Notes from the Patient Navigation Menu.  All visits will list and any notes taken in the Care Plan Note section of the program will display with the visit.

TIP: This is a great place to look for changes in the care plan over time as well as an audit tool to verify if the care plan has been reviewed/updated.
